Thank you for those who have responded however, we have had limited feedback in the first week so we really need your help

Why we need feedback and how it will help sponsors

As mentioned in the previous Circular, now that the Prostheses List management System (PLMS) has been in place for a full listing cycle we are keen to hear your feedback.  The department has developed a ‘PLMS Implementation Feedback Form’ which will provide you with the opportunity to comment on the new online application system.

Accessing the Feedback Form

It is not necessary to identify the sponsor or organisation for the purpose of feedback so no names are required.  The feedback form will take approximately 10-20 minutes to complete.

Timeline for Feedback

The online feedback form will be available for approximately two more weeks, closing on Wednesday 7 September 2016 .  Please do not delay in providing feedback on the PLMS to ensure that we receive your views.
