If you have difficulty viewing, using and electronically submitting the electronic smart forms, this may be a result of:
- · the version of the browser
· whether the browser supports a PDF plugin
· your browser settings
· the type of browser you are using.
The information below may help you access the electronic smart forms.
Browser versionMake sure your browser is up to date.
Installing the Adobe Acrobat ReaderYou will need Adobe Acrobat Reader to be able to view, use and submit the electronic forms. The latest version can be downloaded free of charge from the Adobe website.
You will also need the browser extension for the browser you are using. When you click on the link to download Adobe Acrobat Reader, you will see a screen with a check box to install the relevant browser extension (below). Make sure you check the box for the browser extension. You do not need to check the optional offers boxes.
Once you have installed Adobe Acrobat Reader and the browser extension, return to the forms page and click on the form you want to fill out.
Changing your browser settingsYou may also need to change your browser settings to be able to access the electronic smart farms. However, if your browser settings have been locked by an administrator, this may not be possible.
For help changing your browser settings, you can view Adobe’s browser configuration instructions.
Using the browser extensionWhen you click on one of the forms, you may see a screen with the heading “Please wait…” If the form you have chosen does not appear after a short time, click the “Open in Acrobat” icon on the right hand side of the screen. This will open the smart form in Adobe Acrobat Reader, where you can complete and submit the form.
If you have followed these instructions and you still cannot access the electronic forms, you will need to use the manual (printer friendly) PDF forms. You can send the completed forms to the National Cancer Screening Register by:
